How to Write an Email Asking for Sick Leave

  • The name and contact information of the doctor who recommended the leave or a medical certificate (optional),
  • Any other pertinent information.

You are not required to give any personal information regarding your illness. But if your condition is infectious and impacts the rest of the team, it is usually a good idea to notify your supervisors.
